The Hale Richlen Center for Psychiatry

Each of the providers listed below contract independently with insurance companies.  We recommend all patients contact their insurance company prior to their appointment to verify benefits and prior authorization requirements.  The providers do not accept Medicare or Medicaid. 

 

Patients with Blue Cross/Blue Shield insurance or whom are self pay are required to pay at the time of their appointment.

 

Barbara Hale-Richlen, M.D., F.A.P.A. accepts all insurances as an out of network provider.  Our billing department will submit her patients' claims to insurance companies.  We request patients check with their insurance company to confirm their out of network benefits prior to scheduling their appointment.

Lisa Johnson, MSW, LCSW is an in network provider for Aetna, Cigna, Lyra, United Healthcare and UMR insurance and patients who are self-pay.  She accepts all other insurances as an out of network provider. Our billing department will submit her patients' claims to insurance companies. We request patients check their in-network and out of network insurance benefits to confirm their coverage prior to their appointment.

Melissa Fockel, MSW, LCSW, RPT is an in-network provider for Lyra, United Behavioral Health, United Healthcare and UMR insurance, as well as self-pay patients.  She accepts other insurances as an out of network provider.  Our billing department will submit her patients' claims to insurance companies. We request patients check their in-network and out of network insurance benefits to confirm their coverage prior to their appointment.
